Gallbladder Disease: Cholelithiasis and Cholecystitis

The gallbladder is a small, pear-shaped, sac-like organ located beneath the liver that plays a critical role in the digestive process by storing and concentrating bile. While seemingly minor, gallbladder dysfunction is one of the most common reasons for emergency surgical consultation. When the chemical balance of bile is disrupted, it forms precipitates known as gallstones, which can lead to inflammation, infection, or structural blockage.

Types of Gallbladder Disease

The majority of gallbladder issues stem from cholelithiasis, the presence of gallstones. Gallstones are typically composed of cholesterol or bilirubin. While many people carry stones that never cause an issue, problems arise when a stone migrates and becomes lodged in the cystic duct. Depending on the severity, these are called:

  • Biliary Colic – a temporary obstruction. The gallbladder contracts against a stone, causing intense, episodic pain, usually after a high-fat meal. Symptoms resolve within a few hours.
  • Acute Cholecystitis – when a stone remains lodged, leading to persistent inflammation and potential bacterial infection of the gallbladder wall.
  • Choledocholithiasis: This is a more complex scenario where a stone escapes the gallbladder and enters the common bile duct, potentially causing jaundice or pancreatitis. This can lead to a serious infection cause cholangitis.
  • Biliary dyskinesia – this is found with or without stones. It’s related to the gallbladders ability to “squeeze” out the bile, lack on contraction can lead to distension and pain often mimicking biliary colic.

Risk Factors and the “Four Fs”

While gallbladder disease affects all demographics, women are twice as likely as men to develop stones because of estrogen. High levels of estrogen, whether from pregnancy, hormone replacement therapy, or oral contraceptives, can increase the cholesterol saturation in bile and affect how well it empties. During pregnancy, elevated progesterone levels can also cause the gallbladder walls to “relax” allowing bile to accumulate and form into sludge and stones. Many women experience symptoms during pregnancy, often chalked up to reflux. The third trimester and post-partum periods tend to be when most symptoms arise.

There is an old mnemonic called the “Four Fs” that help to identify the primary risk factors for cholesterol gallstones: Female, Forty, Fertile, and Fat (Obesity). While it’s not the most eloquent mnemonic, it does stick in the memory. Beyond that, rapid weight loss, a sedentary lifestyle, and certain genetic predispositions can also significantly increase the likelihood of stone formation.

Recent research also has highlighted that rapid shifts in weight can cause the liver to secrete extra cholesterol into bile, which then crystallizes into stones. This is something that should also be considered if you are using or planning to use GLP-1s for weight loss.

Signs, Symptoms and Confirming a Diagnosis

The hallmark symptom of gallbladder distress is postprandial pain (pain after eating), specifically in the upper right quadrant of the abdomen. This pain often radiates to the right shoulder or between the shoulder blades. Other symptoms include nausea, vomiting or diarrhea. It can also mimic cardiac symptoms such as chest pain, so do not delay getting evaluated by a physician should you have any of these symptoms.

To confirm a diagnosis, there are a few imaging studies that can help us:

  1. Right Upper Quadrant Ultrasound: This is the most effective and non-invasive tool way to identify stones and measure gallbladder wall thickening or fluid which are signs of inflammation.
  2. HIDA Scan: If an ultrasound is inconclusive but symptoms persist, we use this nuclear medicine test to evaluate the functional gallbladder “ejection fraction” (how well it squeezes out bile and contents) and check for ductal obstructions.
  3. CT scan: While less sensitive for stones than ultrasound, a CT is valuable in emergency settings to rule out complications like gallbladder perforation or abscess.
  4. MRI (MRCP): useful when there is concern for choledocholithiasis

Treatment and Surgical Intervention

Because the gallbladder is not an essential organ, the gold standard for any problems arising with it is to surgically remove it, a procedure called a cholecystectomy. And while there are medications that can “dissolve” stones, this is rarely done because the recurrence rate is nearly 100%. The stones will always come back because the gallbladder itself is the “stone-making factory.”

Most cholecystectomies are performed in a minimally invasive technique using a laparoscopic or robotic approach. In a trauma or complex acute setting, we may occasionally need to convert to an “open” procedure to ensure patient safety.

The prognosis is typically excellent, and afterwards, the liver continues to produce bile, which then flows directly into the small intestine, and most patients return to a normal diet and activity within a few weeks. Some patients will experience diarrhea after surgery, which is usually self-limiting. Others will note intolerance to certain foods such as greasy or fried foods.

A word of caution, the longer you have symptoms, the more inflammation is present, making surgery at times more difficult. If you begin to experience any of the above symptoms, please seek medical advice for prompt evaluation and treatment.
