It feels like you can’t scroll through a feed or sit at a dinner party lately without hearing about “the shot.” GLP-1 agonists, the class of drugs including semaglutide and tirzepatide, have taken the world by storm, transforming from specialized diabetes treatments into a cultural phenomenon for weight loss. While the sheer popularity is undeniable, let’s discuss the science behind how we lose that weight, which is much more critical than the number on the scale.

What exactly is GLP-1?

Glucagon-like peptide-1 (GLP-1) is actually a hormone your body produces naturally in the gut. Its primary job is to tell your pancreas to release insulin after you eat, which keeps your blood sugar in check. However, it also sends signals to your brain, specifically the hypothalamus, to reduce your appetite by signaling that you are full. It essentially slows down gastric emptying, meaning food stays in your stomach longer, and quiets that mental noise whispering constantly about food.

From Prescription to “Off-Label” Fame

Originally, these medications were FDA-approved for Type 2 diabetes. However, when the profound weight loss effects became clear, new versions were fast-tracked specifically for chronic weight management in patients with obesity or weight-related medical conditions.

Today, we see a mix of:

  • On-Label Use: For patients with clinical obesity (BMI >30) or those with a BMI >27 plus a comorbidity like hypertension.
  • Off-Label Use: Individuals seeking “vanity” weight loss or cosmetic shedding of 10–15 pounds. This surge in popularity has led to global shortages and a massive conversation about who “deserves” access to these medications.

First, The Good:

The health benefits of GLP-1s often go far beyond just looking better in jeans. By reducing visceral fat (the more unhealthy fat that you can’t see that’s stored around your organs), these drugs have shown to be effective at improving cardiovascular health and reducing the risk of heart attacks, reversing pre-diabetes, lowering inflammation and potentially having brain benefits, which researchers are currently investigating for conditions like Alzheimer’s.

The Catch: Muscle vs. Fat

Here is where we need to take a pause though… Is all weight loss created equal?

When you lose weight rapidly on a GLP-1, you aren’t just losing fat. Studies have shown that a significant portion, sometimes up to 25-40%, of the weight lost can actually be lean muscle mass. And that’s not good.

From a functional perspective, muscle isn’t just for being strong or looking “toned”. It is the body’s metabolic engine and the primary storage for glucose. And rapid weight loss can also impact bone mineral density. Plus, losing too much muscle leads to a lower metabolic rate and, in more extreme cases, a dangerous condition called sarcopenic obesity, where you are thin and weak but metabolically “fat.”

How To Preserve Bone and Muscle Health

If you are on GLP-1s, the goal shouldn’t just be “less of you”, it should be a healthier version of you. Some ways to protect your muscle and bone health while losing extra pounds, is by being intentional about your journey.

Prioritize protein. Because your appetite is suppressed, you’re consuming less calories overall, so make sure that every bite counts. Aim for high a high protein intake (1.2 to 1.5 grams per kilogram of body weight) to provide the building blocks your muscles need to survive the calorie deficit.

Resistance training gives your muscles a reason to stay. Think of it as your body saying “we still need this muscle so don’t burn it for fuel”. Lifting weights or doing bodyweight workouts will help keep your muscles not only existing, but strong.
